Volunteer Transportation Request eForm
A new eForm version of the Volunteer Transportation Request has been developed, tested and is now available for use by our volunteers.
First introduced in July 2015, our eForm platform provides a streamlined way to submit various forms and reports to program staff. Featuring dynamic fields and controls, eForm is easy to use on any device at home or out in the field.
The Volunteer Transportation Request is available for volunteers who wish to avail themselves of Florida Statute § 39.8296(2)(b)(7), which permits guardian ad litem volunteers to transport children represented by the Florida Guardian ad Litem Program.
The passage of this law in 2012 was a great step forward for the advocacy of children. We encourage all volunteers — who wish to do so — to apply to transport children by submitting the new request eForm and downloading the Transportation Guidelines in the Standards and Guidelines section of the Form and Document Library.
